The Community-Inclusion Connector Award is presented by the New Jersey Association of Community Providers (NJACP) to recognize a community-based organization or business that creates meaningful opportunities for people with disabilities to connect, participate, and belong within their communities.

This award honors an organization whose program, service, or initiative intentionally promotes inclusive engagement, enhances quality of life, and lifts individuals up through relationships, access, purpose, and participation. The recipient goes beyond providing services by actively breaking down barriers, fostering natural supports, and creating welcoming spaces where people with disabilities are seen, valued, and empowered.

NJACP seeks to recognize a community-based organization that:

• Builds bridges between people with disabilities and the broader community

• Creates inclusive opportunities for social, civic, cultural, recreational, or educational participation

• Promotes independence, confidence, and self-determination

• Strengthens community understanding, acceptance, and belonging

The successful nomination will highlight how the organization’s efforts have positively transformed lives, demonstrating measurable or meaningful outcomes that reflect increased inclusion, connection, and opportunity for people with disabilities. At its core, the Community-Inclusion Connector Award celebrates organizations that lift individuals up by ensuring they are not just present in the community, but truly part of it.

It is important to note that only complete nomination packets that are received at the NJACP office no later than Monday, February 9, 2026, 11:59 p.m.., will be considered.  The nomination packets must include all written materials as well as the signature of the agency Executive Director/CEO as well as letters of support.
